The common pattern is stomach trouble

In the Zepbound weight trials, nausea was reported by about 25 to 29 in each group of 100 given tirzepatide, against 8 in each 100 given a dummy injection. Diarrhea, vomiting, and constipation were also common.

These problems appeared most often while trial doses were being increased. That is a trial finding, not a dosing direction.

Problems the label treats seriously

Fluid loss and kidneys

Severe vomiting or diarrhea can lead to dehydration and kidney injury.

Pancreas and gallbladder

The label warns about pancreatitis, a painful swelling of the pancreas, and sudden gallbladder disease. A warning sign of pancreatitis is stomach pain that is severe or will not go away, sometimes reaching through to the back.

Allergic reaction

Serious allergic reactions have been reported, including anaphylaxis, a sudden, severe allergic reaction that is an emergency.

Slow stomach emptying

Zepbound is not recommended for people with severe gastroparesis, a diagnosed condition in which the stomach empties very slowly. If a doctor has ever told you that you have it, mention it.
